Lower back pain affects around four in five adults at some point in life. The good news is that most cases are not serious, and most people respond well to a conservative, evidence-based approach.

What is lower back pain?

Lower back pain is discomfort in the region between the bottom of the ribcage and the top of the pelvis. It can feel like an ache, stiffness, tightness or a sharper catch when you move. Most cases are related to how the muscles, joints and discs of the lower back are moving and loading — not to serious underlying disease.

Common symptoms

  • A dull ache or tightness across the lower back
  • Sharp discomfort when bending, twisting or standing up
  • Stiffness in the morning or after long periods of sitting
  • Discomfort that eases with gentle movement

Common contributing factors

The lower back carries a lot of load and moves in every direction. Long hours of sitting, sudden lifts, poor sleep, low activity levels or a build-up of small stresses can all leave the spine and surrounding muscles under-supported. Over time, joint restriction and muscle guarding can create the familiar tight, achy or sharp discomfort.

When to consider seeing a chiropractor

  • Discomfort that has lasted more than a few days or keeps returning
  • Stiffness in the morning or after sitting
  • Discomfort that limits work, sleep or exercise
  • Recurring flare-ups after lifting, gardening or driving

Seek urgent medical care if lower back pain follows a significant trauma, or is associated with fever, unexplained weight loss, numbness in the groin or loss of bladder or bowel control.

Simple self-management tips

  • Keep moving — gentle walking is usually better than complete rest
  • Alternate sitting and standing throughout the day
  • Bend from the hips and knees when lifting, keeping loads close to the body
  • Use heat for tight, achy muscles and short bouts of ice for sharp flare-ups
